The recent white paper Equity and excellence: Liberating the NHS demonstrates a commitment to long term conditions stating that they will “introduce choice in care for long-term conditions as part of personalised care planning from 2011.”
“Commissioning by GP consortia will mean that the redesign of patient pathways and local services is always clinically-led and based on more effective dialogue and partnership with hospital specialists. It will bring together responsibility for clinical decisions and for the financial consequences of these decisions. This will reinforce the crucial role that GPs already play in committing NHS resources through their daily clinical decisions – not only in terms of referrals and prescribing, but also how well they manage long-term conditions, and the accessibility of their services. Through a series of presentations this one day conference chaired by Dr Shahid Ali GP and Deputy Director GP Provider Services Bradford and Airedale PCT, provides important updates in the latest developments in Long Term Conditions and the care planning commitment released by the Department of Health on the 4 March 2010.
The conference opens with a keynote presentation from Dr Steven Laitner General Practitioner Consultant in Public Health Medicine and Associate Medical Director East of England Strategic Health Authority and Clinical Advisor to 18 Week National Implementation Team Department of Health, who will discuss developing care plans for Long Term Conditions, putting people at the centre of decision making about their care and the progress in meeting the December 2010 deadline.

This one day tenth annual conference focuses on the redesigning processes, change and innovation to improve the patient journey. Following the chairman’s introduction, Mr Rashad Massoud Senior Vice President Quality and Performance Institute will deliver a presentation on redesigning the patient journey, discussing process mapping and redesign, Quality, Innovation, Productivity and Prevention, mapping and analysing the patient journey to identify areas for improvement, and does improving quality across the patient pathway save money.
The morning continues with an extended interactive session from Mr Jonathan Hill Head of Quality Assurance and Care Management 2gether NHS Foundation Trust, who will discuss planning and preparing a Process Mapping exercise, understanding the jargon, symbols and shapes, allocating value to the process map, tips for ensuring the exercise provides the information you need to understand the patient journey and how to identify areas for improvement.
Two facilitated workshops will run alongside the main conference programme. The first lean thinking and redesign will focus in lean healthcare and the essentials for success. The second, in depth Process Mapping and redesign, an interactive workshop, which will take delegates through the Process Mapping process with a hands on experience through the use of case study exercises.
